You have now entered a study examining your perspectives on health. Please note that you must be 18 years of age or older, and living in Canada to participate in this study.

During this study you will be asked to complete questionnaires assessing your attitudes and beliefs about various health related situations. This study is expected to take approximately 45 minutes to complete. You will receive a gift card of $5.00 as a token of our appreciation for your participation. If you wish, you may pause during the completion of the questionnaire and return at a later time to complete the survey, but you must remember your login user ID and your password. For privacy purposes if your Internet browser is idle on one page for 5 minutes or more, you will automatically be redirected to the Carleton home page, and your information on that page will not be saved. You can come back later to complete it, but you need to log in with your user ID and password. We ask that you try to complete the survey within the same day of starting it.

Please complete all questionnaires truthfully. In order to guarantee the validity of the data, we will be performing validity checks (i.e., answers will be checked to ensure they were not given at random). If the responses are deemed to be random, a gift card will not be issued.

This online study has been designed to be viewable using Internet Explorer^® as the primary browser. Although its operation will operate over multiple browser environments, some rendering effects and the design layout may not be displayed as intended. We highly recommend using Internet Explorer^® for viewing this site. The following browser are supported.

Due to the ease of producing random responses, before sending you the gift card, validity checks (i.e., answers will be checked to ensure they were not given at random) will be used to determine the sincerity of your responses. Only those participants who have taken the time to read the questions and provide us with valid responses will be sent a gift card. In the event that an individual has responded at random, we will NOT honour the gift card for that participant. Please note that you are only allowed to participate in this study once. You will not receive a gift card if you participate more than once.
